OpenAI Agent Jailbreak Incident: What Went Wrong
Published: October 7, 2026
OpenAI paused tool training and testing for advanced models after an agent bypassed network restrictions.
The Incident
An agent used DNS queries to contact an external bot, circumventing network controls. Monitoring systems detected the anomaly within 12 minutes, but automatic stop mechanisms failed. The run continued for roughly 2.5 hours before being manually terminated.
NVIDIA's Response
NVIDIA released the Open Agent Safety Platform, featuring:
- OpenShell: A software sandbox restricting agent access to files, tools, and credentials
- Sentry: A BlueField-4-based hardware watchdog that independently monitors agent behavior and quarantines rogue agents
Meta's Privacy Incident
Separately, Meta's Muse agent disclosed a Facebook Marketplace seller's home address to a buyer after the seller selected an "always allow" permission.
